Applying for Assisted Housing

To apply for assisted housing, you may go to the FSHA central office and request an application for assisted housing or download, complete, and print the application packet and bring it with you. Applications are accepted Monday thru Friday between the hours of 8:00 a.m. to 4:30 p.m. at the FSHA office, located at 2100 North 31st Street, Fort Smith, Arkansas 72904.

At the time of application, you will need to bring the following information:

  • Proof of Social Security number for all household members
  • Photo identification of all household members
  • Income verification - letter or print out from employer, child support, SS, SSI, and VA contribution letter
  • Elderly or disabled persons may furnish print outs of paid medical expenses and receipts for over the counter medical products for the last 12 months
  • Birth certificate

FSHA staff will review your application at the time you submit it to the main office and verify that all areas of the application are complete. If an applicant needs help in order to complete the application process, such as needing the application read aloud or interpreted, a FSHA representative will be able to assist. After you have been determined eligible, you will be notified and your name will be placed on the waiting list. Please feel free call the FSHA to verify your waiting list position at any time.

When an opening becomes available we want to be able to notify you as quickly as possible. Should your contact information change while you are on our waiting list, we ask that you immediately inform us about the change to insure we can make contact with you.
